PLUS The Application and Promissory Note required to obtain a PLUS education loan.
Grad PLUS Addendum This is an addendum to the Federal PLUS Loan Application and Master Promissory Note (MPN) signed by the borrower of the Federal PLUS Loan.
Stafford MPN (Master Promissory Note) The Master Promissory Note required to obtain a Stafford education loan.
Consolidation 180 Day Add-On Provision Use this form to request that your lender add an eligible education loan(s) that was not originally included.
Consolidation Cancellation Request If you have started the consolidation process with another lender and wish to cancel the consolidation application, complete this form and submit it to your current consolidating lender.
Temporary Total Disability Deferment Request Form Deferment request for the qualified borrower who is or whose spouse or dependent is temporarily totally disabled. (Available to a borrower who has an outstanding balance on a loan made prior to July 1, 1993 only.)
Total and Permanent Disability Request Form Necessary request form for loan discharge due to total and permanent disability. This form requires physicians certification of the disability.
Education Related Deferment Request Form Deferment request for borrowers who are engaged in a Graduate Fellowship, Rehabilitation Training, or Internship/Residency Program, or are teaching in a designated Teacher Shortage Area.
Parental Leave/Working Mother Deferment Request Form Deferment request for mothers who have an outstanding balance on a loan made between July 1, 1987 and July 1, 1993 and who meet specific qualifications regarding pregnancy, child care, school enrollment, and employment.
Economic Hardship Deferment Request Form Deferment request for the qualified borrower who has been granted this deferment by the William D. Ford Direct Loan Program, who is receiving federal or state public assistance, who is serving as a Peace Corps volunteer, or who meets specific low income qualifications. (Available to a borrower who has an outstanding balance and all loan(s) were made on or after July 1, 1993.)
In School Deferment Request Form Deferment request for qualified borrowers who have returned to school on at least a half-time basis.
Military Deferment Request Form Deferment request for qualified borrowers who are in the military.
Public Service Deferment Request Form Deferment request for the qualified borrower who is on active duty in theArmed Forces or National Oceanic and Atmospheric Administration, is an officer of the Public Health Service, is serving in the Peace Corps, or is a paid volunteer in the Action Programs or for a Tax-Exempt Organization.
Unemployment Deferment Request Form Deferment request for the qualified borrower who is unemployed but seeking work in any field.
Temporary Hardship Forbearance Form A forbearance is an option available to students who are not eligible for a deferment. If you are financially unable to make your student loan payments, you may be eligible for this temporary suspension or reduction of payments.
Mandatory Forbearance Form A forbearance may also be granted if federal student loan debt exceeds 20% of the borrower's gross income. Forbearance will not eliminate any prior derogatory credit history. Before you utilize these request forms, please contact the our Customer Servicing Department toll-free at 1-800-366-4372. Your student loan holder needs to be aware of your situation in order to provide you with the appropriate information and instructions.
